Application Checklist
- Student Information Sheet Part I Download or Submit Online
- Student Information Sheet Part II Download or Submit Online
- Emergency Information Sheet
- A copy of the student's eighth-grade report card (for upperclassmen, an official or unofficial copy of their transcripts)
- Results of the SMHS Scholarships and Admissions Test or a copy of the student's most current standardized test scores - i.e., Iowa, California, or CSAP)
- A personal letter of recommendation - the family decides whom to ask for the recommendation letter. The letter can be written by a priest, teacher, counselor, principal, or whomever the family would like to ask.
- A teacher recommendation - please use the form provided
- A one-page, handwritten essay from the student explaining why he/she would like to attend SMHS
- The Annual Registration Fee (non-refundable) is on a graduated scale based on the date you apply. If your application is received:
- Before March 15 - $300
- Between March 16 and April 15 - $350
- After April 16 - $400
- Upperclassmen will be interviewed once their application has been received
- Upon acceptance to St. Mary's you will have the opportunity to select your area of interest for 30 volunteer hours through our Pirates Parents Crew
Upon Acceptance
When all application materials have been received and the results of the SMHS Scholarships and Admissions Test are available, the Admissions Committee will meet to determine the applicant's status. All applicants will be informed in writing of their status as either accepted, partially accepted, accepted on a probationary status, or not accepted.
Once accepted, the student's family will need to provide the following items in order to complete the registration process:
- A completed Tuition Contract
- A completed Course Selection Sheet
- A completed Parent Volunteer Pledge
- A copy of the student's Immunization Records
